A/V Services Electronic Request Form Print

Electronic forms must be submitted at least 2 business day before the requested date of use. Individuals who request items less than 2 business days in advance must make a "face-to-face" request in Rosenau Hall Room 233. Please note: During "face-to-face" request, allow at least 15 minutes to identify availability & data entry into schedule. Request via telephone are only permitted for "semester-long" requests.

STEP 1: Review Audio / Visual Services Guidelines

STEP 2: Click on Electronic Request Form located at the bottom of the page and complete form.

STEP 3: Within 1 business day, you will receive an e-mailed confirmation of your pick-up/delivery times.

GUIDELINES FOR EQUIPMENT USE

  • All equipment is for use by the School of Public Health students, staff, and faculty only. This includes the following departments: Dean's Office, External Affairs, IIS, BIOS, ESE, EPID, HBHE, HPAA, MCH, NUTR, PHLP and Center for Public Health Practice.

  • All equipment is reserved on a first come, first served basis. Advanced reservations are strongly recommended to ensure equipment availability. To better serve the School of Public Health, all equipment must be returned by the specified time. If you find that you need to keep an item longer than anticipated, please notify our office as soon as possible. If items are available, we will attempt to accommodate your request. Check-out equipment should not exceed a 1 week period.

  • All equipment must be picked up or returned between the hours of 8:00 AM and 5:00 PM. It is not acceptable to leave equipment outside of the office door.

  • Reserve equipment only for the actual scheduled hours of your class or meeting. (i.e., If you have a room reserved from 9:00 AM to 11:00 AM, do not request equipment to be placed in the room at 8:45 and removed at 11:15. Submit 9:00 AM and 11:00 AM.) If you need equipment well in advance of your class or meeting time, please reserve the room earlier through the Office of Student Affairs.

  • Projectors and PC's must be turned off after use or when not being used for periods of a half-hour or more. You are responsible for communicating this rule to guest lecturers or students in your class who may use the equipment.

  • No unauthorized software will be allowed on SPH A/V Services computers. We recommend that that you use flopy; Zip 100 disks or CD-R's (not CD-RW's) to bring your files to class. Do not leave documents on the desktop. Any unauthorized software or documents left on A/V Services computers are subject to deletion at any time.

  • Please inform A/V Services of any cancellations as soon as possible. If a reservation is not cancelled, records will indicate a "No Show". Repeated No Shows will result in limitations or termination of future equipment use.
