Video Conferencing Reservation Form

To reserve dates for the Mayes Telecommunication Center (230 Rosenau Hall) or the Cooper Building Facility, please follow the procedures below: Contact the Mayes Center Scheduler to identify available dates and vidoeconference facilities. If dates are not available, Instructional Media Services will attempt to identify alternative dates for your activity. Phone: (919) 966 - 7082   E-mail: mayes-schedule@listserv.unc.edu  Once an available date has been identified by the scheduler, click on Access Mayes Center Reservation Form located at the bottom of the page. Complete the reservation form and submit online. Upon receipt of your reservation form, you will receive a Mayes Center Confirmation Sheet via e-mail that will reflect the details of your activity. Please allow at least  5 business days to receive your intial confirmation.  The date for your activity is not scheduled without submission of your reservation form and receipt of the confirmation sheet. For those reservations using sites not listed on the form, you will be required to submit the technical information for those locations on the "Outside Sites" section of the form. Changes to your reservation must be made no later than 10 business days before your scheduled event. Changes requested after that time are not guaranteed. You can make changes to your reservation by selecting the "Revisions to Existing Form" option.  You must use your Request ID Number that will be issued to you after completing your intitial request.  Reservations are taken on a first come first serve basis. It's strongly recommended that you submit the reservation form immediately after you & the Mayes Center Scheduler have identified a date to ensure confirmation.
